Doorknob Portland Feedback Prize Rules and Disclaimer

Prizes: There will be (1) $1000 Check awarded once a month by Doorknob Enterprises, LLC or Doorknob Ventures L.L.C. (“Doorknob”) in the Portland, Vancouver Area. *

Each of the 16 Jiffy Lube Locations owned by Doorknob Enterprises, LLC or Doorknob Ventures L.L.C.(“Doorknob”) in the Portland, Vancouver Area will be giving away (2) Jiffy Lube Signature Service Oil Change. The Signature Service Oil Change can be redeemed at any Doorknob owned Jiffy Lube Location. See web-site for locations.

Prizes will be awarded starting on Wednesday, December 1, 2010 and will be randomly selected from the previous month’s survey entries from each Doorknob owned Jiffy Lube location.

Eligibility:

Jiffy Lube Feedback promotion is open to all Oregon and SW Washington Residents, 18 years of age or older. Employees of Doorknob, Jiffy Lube International, Inc., or its franchisees/joint venture partners, Shell Oil Company, Pennzoil-Quaker State, Inc. their affiliates, advertising and promotion agencies, and the immediate family members of each are not eligible to win. One entry per invoice, persons are not eligible to win more than one prize in a calendar year. No Purchase is necessary to win.

Doorknob has the right to cancel, modify or suspend the promotion at anytime with-out notice.

This offer is void where prohibited by law and is subject to all Federal, State and local laws and regulations.

Consent:

By entering you authorize the use, without additional compensation of his or her name and/or likeness and/or photograph for promotion and advertising purposes in any manner and in any medium (including without limitation, radio broadcasts, newspapers and other publications and in television or film releases, slides, videotape, distribution over the internet and picture date storage) which Doorknob may deem appropriate

* Form 1099MISC must be issued to U.S. individuals that receive awards, honorariums, and prizes in the calendar year that total US $600 or more. 1099s are not issued for scholarships and fellowships. Doorknob Enterprises L.L.C. is responsible for providing the information, for the preceding year, that would be needed to complete Form 1099MISC. The IRS requires the 1099MISC forms to be issued no later than January 31st. When the customer is informed of the winning of the prize they will be presented with Form W-9 (PDF, 98 KB) to obtain the individual's names, addresses, and social security numbers. This form will need to be completed to collect the prize.
